Communications Equipment / Support & Training Request Form

Complete the following form to request corporate communication assets to support a planed Official CAP function such as an Airshow or SAREX.

You will be contacted by a member of the Communication Support Team in your area to fill your request.

NOTE: Your request for Communications Support MUST be submitted no later than 30 days PRIOR to your event.

I agree and understand that the OIC / Requestor is fully responsible to return all loaned equipment in the order which it was received.

I agree to return all loaned equipment to the issuer within 48 hours after the activity or event.

I understand that Wing level official functions and Missions take precedence over Group and Squadron Level activities.

I understand that Communication Assets may not leave the Wing without written consent of the PAWG DC.
